Support plan evidence guidance

Providing evidence for your support plan

To set up a support plan, the Wellbeing and Disability Support team will need evidence of your disability, health condition, or specific learning difference.

Providing evidence helps us understand how your condition affects your studies, so appropriate adjustments and support can be recommended.

If you’re unsure what to provide or don’t yet have documentation, contact the team at wellbeing@rcl.ac.uk for advice and guidance.

What documentation to provide

Evidence should come from a qualified professional who can describe your condition and explain how it affects your day-to-day learning.

This information allows the Wellbeing and Disability Support team to tailor your support and ensure adjustments are fair, appropriate, and effective.

Acceptable evidence includes:

  • A medical letter or report from a GP, consultant, or specialist confirming your diagnosis or condition
  • A report from an educational psychologist or specialist assessor for a Specific Learning Difference (SpLD), such as dyslexia, dyspraxia, or ADHD
  • A letter from a registered mental health practitioner or counsellor confirming an ongoing mental health condition.

Documentation should ideally:

  • Be written in English (or include a certified translation)
  • Be signed and dated by the professional
  • Describe the functional impact of your condition – for example, how it affects studying, learning, or attendance

Tip: If your evidence was used for a Disabled Students’ Allowance (DSA) application, you can submit a copy of the same report. The Wellbeing and Disability Support team can use this information to create your support plan.

What if you don’t have evidence?

If you don’t currently have medical or diagnostic evidence, the Wellbeing and Disability Support team can help with:

  • Advice on what type of documentation is suitable for your situation.
  • Support you in accessing an assessment service if you think you may have an undiagnosed learning difference.

In some cases, temporary support may be put in place while evidence is being gathered, so you don’t miss out on essential adjustments.

Students are encouraged not to delay contacting the team. The earlier Regent College London is aware of your circumstances, the sooner support can begin.

Submitting evidence and confidentiality

Submit your evidence by:

  • Emailing it securely to wellbeing@rcl.ac.uk.
  • Bringing it to a one-to-one appointment with a member of the Wellbeing and Disability Support team.

All medical and diagnostic information is stored securely and treated as confidential, in line with data protection regulations.

Information is only shared with staff who need to know in order to implement agreed adjustments, and only with your informed consent.
